The importance of a primary care provider to manage blood pressure  care will be emphasized and referral to a social worker within the Healthy Libraries team will be provided should the patron require this resource. Additionally, patrons can borrow a free Blood Pressure Loaner Kit  from the library (if one is available). The kit includes a American Heart Association validated self-measured blood pressure monitor. Please talk with library staff about borrowing the BP Loaner Kit.
