PRE-ORIENTATION REQUIREMENTS

The following is a list of requirements that need to be
completed by prior to the MPH orientation:

1.  Any outstanding conditions outlined in your
admission must be met.

2.  Students who do not have a clinical degree,
or are not in a clinical degree program, must provide certificates of
completion for the following two online courses:  Anatomy and Physiology 101 and Medical
Terminology 101.

The
courses are offered through this website http://www.universalclass.com.

3. Complete the online HIPAA training by following the
instructions ound on the website of the Office of the Vice President for
Research: http://ws.cc.stonybrook.edu/research/orc/humans/training.shtml

4. Complete the online Protection of Human Subject training
offered by the Collaborative Institutional Training Initiative (CITI) at:
http://www.citiprogram.org.

5.  Complete online Math Placement Exam through
the following website:  http://assess.math.sunysb.edu/welcome.pl

Once
admitted you will received additional instructions on how to access this exam.
Also, if you wish to prepare for biostatistics (HPH 506 & 507), the program
recommends the following on-line course that will provide a math refresher:
Math All-In-One from http://www.universalclass.com.

6. Complete the Health Form and send to Student Health Services.

7. Complete the MPH Student Profile.

8. Complete online Competency Assessment Survey. Once admitted
you will receive the link to the survey as part of your eWelcome Packet.
